I never understood the concept of fashionably late. 

If I was ever late, it was rude or disruptive.  Ab inconvenience or insult to others,  Fashionable was never among the adjectives.

It’s a way of showing off that your time is more valuable than their time, by forcing them to meet your schedule instead of you meeting their schedule.

Modern businesses seem to work on that principle regarding meeting, at least.

Generally, you have to be perceived as having enough status/clout that you can get away with wasting other people’s time in such a manner.

It depends on how you define clout.  Very low-level functionaries seem to take pleasure in doing this.  See most DMV , for example.  I understand that dealing with civil servants of any level in many countries can be bad.  My only non-American example was in West Germany, and they lived up to the stereotype of German punctuality.  They called me in to the office 30 seconds before the interview/test was supposed to take place.  Small talk took less than a minute, then I was given the written test for my driver's license.

I've been lucky with my doctors.  Assuming I'm not a walk in, if I get to the clinic a little early, I get seen a little early.  Due to some strangeness with my transportation, I got to my last appointment about 45 minutes early.  I was done with the whole thing by the time it was scheduled for.  My appointments with my urologist have always been exactly on time.
